An FAA-requested psychiatric evaluation is not a normal office visit. It is a records-heavy, documentation-heavy assessment with your medical certificate on the line. Here is what yours includes.
Your FAA letter, MedXPress history, treatment records and any prior evaluations, read properly before you ever sit down. The referral question drives everything.
A full psychiatric evaluation with time to tell the whole story: history, treatment, recovery and where you are now. No 10-minute rush.
Validated rating scales and objective testing when the referral question calls for them, so findings rest on data as well as clinical judgment.
Organized findings and recommendations, stated plainly and prepared for the people who will read them: you, your AME and the FAA.
Send the short form, or text (904) 664-3219 or call (904) 705-9335. Within one business day the office replies with your quote, next steps and a secure link for your letter and records.

“As an independent FAA/HIMS psychiatric evaluator, I have two responsibilities: your well-being, and the safety of the national airspace. Holding both is what makes a report credible to the FAA, and it’s what serious aviators want.”Dr. Raj Loungani, MD, MPH, FAPA
